* Fill out the form, being sure to sign and date the Release at the bottom.
* Fax or Mail* your completed form with payment (deposit or full amount).
Payment can be made two ways:
1. Pay the full amount: $850.00 USD ($750 if you register before April 9) -or-
2. Pay a $200 USD non-refundable deposit*** with your registration
and then pay 2 additional payments totaling $650 USD:
a. 1st payment of $325 USD due by March 16
b. 2nd payment of $325 USD due by April 16
